D'oh! Jay's right, the double quotes in the HTMl need to be escaped. Besides
the two forms above, 

        print "<form name=\"a_form\">\n";

will also do the trick. See perldata(1), under "Scalar value constructors"
for more that you even wanted to know about how to quote strings.
